Abstract Objective The purpose of the present study is to compare intraoperative blood loss, operating time, laminectomy time, hospital length of stay, and complications in thoracolumbar spinal decompression using ultrasonic bone scalpels (UBSs) with conventional procedures. Methods Forty-two patients who underwent decompressive laminectomy and pedicular screw fusion with a surgical level of 1-5 levels between February 1, 2020, and June 30, 2022, in a single institution were evaluated for eligibility, and 11 were excluded due to a history of spinal surgery (n= 3), spinal tumor (n= 3), and spinal infection (n= 5). A total of 31 patients were randomly divided into the UBS group (n =15) and the conventional group (n =16). Intraoperative blood loss, operating time, laminectomy time, hospital length of stay, and complications were recorded. Results Intraoperative blood loss and laminectomy time were significantly lower in the UBS group (656.0 ± 167.6 ml, 54.5 ± 27.4 minutes, respectively) than in the conventional group (936.9 ± 413.2 ml, 73.4 ± 28.1 minutes, respectively). Overall operation time, hospital length of stay, and complications were all similar between the groups. Conclusion The UBS is a useful instrument for procedures performed near the dura mater or other neural tissue without excessive heat or mechanical injury. This device is recommended for various spinal surgeries in addition to high-speed burrs and Kerrison rongeurs.


Resumo Objetivo O objetivo do presente estudo é comparar perda de sangue intraoperatória, tempo de operação, tempo de laminectomia, tempo de internação hospitalar e complicações na descompressão espinhal torácica utilizando bisturis ósseos ultrassônicos (BOUs) em relação aos procedimentos convencionais. Métodos Quarenta e dois pacientes submetidos a laminectomia descompressiva e fusão pedicular do parafuso com um nível cirúrgico de 1 a 5, entre 1° de fevereiro de 2020 e 30 de junho de 2022 em uma única instituição, foram avaliados para elegibilidade e 11 foram excluídos devido ao histórico de cirurgia espinhal (n= 3), tumor espinhal (n= 3) e infecção espinhal (n= 5). Perda de sangue intraoperatória, tempo de operação, tempo de laminectomia, tempo de internação e complicações foram registradas. Resultados A perda de sangue intraoperatória e o tempo de laminectomia foram significativamente menores no grupo BOU (656,0 ± 167,6 ml, 54,5 ± 27,4 min, respectivamente) do que no grupo convencional (936,9 ± 413,2 ml, 73,4 ± 28,1 min, respectivamente). O tempo de funcionamento total, o tempo de internação e as complicações foram todos semelhantes entre os grupos. Conclusão O bisturi ósseo ultrassônico é um instrumento útil para procedimentos realizados próximos à dura-máter ou outro tecido neural sem calor excessivo ou lesão mecânica. Este dispositivo é recomendado para várias cirurgias de coluna vertebral, juntamente com rebarbas de alta velocidade e pinça Kerrison.

RESUMO

Describimos esta serie de 15casos programados para descompresión de columna lumbar a un único nivel con instrumental, en la que practicamos bloqueo ecoguiado bajo el músculo multífido (SMFB). Se inyectó el anestésico local en profundidad hacia el músculo multífido logrando el bloqueo seguro de las ramas dorsales de los nervios espinales a múltiples niveles en esta serie. Con ultrasonidos (US) puede identificarse el músculo multífido en los planos axial y parasagital. La punta de la aguja se visualiza fácilmente bajo el músculo multífido y en posición medial hacia el proceso transverso. Se documentó la buena calidad de la analgesia utilizando las puntuaciones para el dolor. No se produjeron episodios adversos. Este bloqueo debe compararse con la analgesia multimodal rutinaria o el bloqueo en el plano interfascial del músculo toracolumbar recientemente descrito, en términos de seguridad y eficacia analgésica.(AU)

In this series, injections of local anesthetic deep to the multifidus muscle provided reliable block of dorsal rami of spinal nerves at multiple levels. With US, the multifidus muscle can be identified both in axial and parasagittal planes. Needle tip is easily visualized beneath the multifidus and medial to the transverse process. Good quality analgesia was documented by pain scores. There were no adverse events. Further studies are needed to compare this nerve block with routine multimodal analgesia or with the recently described thoracolumbar interfascial plane block to compare safety and analgesic efficacy.(AU)

Introducción La mielopatía cervical degenerativa representa una entidad patológica producida por la estenosis del canal medular cervical, resultando en una compresión crónica de la médula espinal, variable y progresiva. El abordaje quirúrgico de la columna cervical puede realizarse por vía anterior y/o vía posterior. Respecto al abordaje posterior, existen 2 técnicas fundamentales: laminoplastia y laminectomía con fijación posterior (LFP). En la literatura actual existe controversia acerca de cuál de las 2 técnicas permite obtener mejores resultados postoperatorios. El objetivo es el estudio de las diferencias entre laminoplastia y LFP desde el punto de vista clínico y radiológico. Materiales y métodosSe realiza un estudio de una cohorte histórica de 39 pacientes (12 LFP y 27 laminoplastia) intervenidos en un período de 10 años en el Hospital Universitario La Paz con un seguimiento de 12 meses tras la cirugía. Se analizan y comparan los resultados clínicos mediante la escala de Nurick y la Escala Japanese Orthopaedic Association modificada (mJOA) y los resultados radiológicos mediante el ángulo de Cobb, eje sagital vertical, T1 Slope y el alineamiento (medido mediante Cobb-T1 Sloppe). Resultados Se observan diferencias significativas en la mejoría postoperatoria de la escala Nurick (p=0,008) y mJOA (p=0,018) en el grupo de laminoplastia. En LFP se objetiva una tendencia a una mejoría mayor, pero no se alcanza la significación estadística debido al bajo tamaño muestral de este grupo. No se objetivan diferencias estadísticamente significativas en cuanto a la variables radiológicas. Respecto al total de complicaciones, se observó un número mayor en el grupo de laminoplastia (7 casos) frente a LFP (un caso), pero no se vieron diferencias estadísticamente significativas... (AU)


Introduction Cervical degenerative myelopathy is a variable and progressive degenerative disease caused by chronic compression of the spinal cord. Surgical approaches for the cervical spine can be performed anteriorly and/or posteriorly. Regarding the posterior approach, there are 2 fundamental techniques: laminoplasty and laminectomy with posterior fixation (LPF). There is still controversy concerning the technique in terms of outcome and complications. The aim of the present work is to analyze from the clinical and radiological point of view these 2 techniques: laminoplasty and LPF. Materials and methods A historical cohort of 39 patients was reviewed (12 LFP and 27 laminoplasty) including patients operated in a 10 years period at the Hospital Universitario La Paz with a follow-up of 12 months after surgery was carried out. The clinical results were analyzed and compared using the Nurick scale and the modified Japanese Orthopaedic Association Scale (mJOA) and the radiological results using the Cobb angle, Sagittal Vertical Axis, T1 Slope and alignment (measured by Cobb-T1 Sloppe). Results Significant differences were observed in the postoperative improvement of the Nurick scale (P=.008) and mJOA (P=.018) in the laminoplasty group. In LFP there is a tendency to a greater improvement, but statistical significance is not reached due to the low sample size of this group. No statistically significant differences were observed in the radiological variables. Regarding the total number of complications, a higher number was observed in the laminoplasty group (7 cases) versus LFP (one case), but no statistically significant differences were observed. (AU)

Objective: To evaluate the incidence of significant intraoperative electrophysiological signal changes during surgical positioning, and to assess the effectiveness of head and neck repositioning on the restoration of signals, among patients undergoing surgery for cervical myelopathy.Material and methods: We used multimodal intraoperative monitoring (somatosensory [SEP] and motor evoked potentials [MEP] and spontaneous electromyography) before and after patients’ positioning in a consecutive cohort of 103 patients operated for symptomatic cervical myelopathy. Significant changes were defined as>50% attenuation in amplitude or>10% increase in latency of SEP, or abolishment or 50–80% attenuation of MEP.Results: Out of 103 patients (34.9% female, median age 54.5 years) 88 underwent laminectomy (85.4%) and 15 (14.6%) anterior approach. At the time of positioning, signal alterations occurred in 44 patients (42.7%), yet only 11 patients (10.7%) showed alarming changes. Immediate neck repositioning of these resulted in complete (n=6) or partial (n=4) restoration of potentials, yielding no postoperative deficits. The patient in which signals could not be restored after repositioning resulted in added postoperative deficit. The accuracy (true positives plus true negatives) of monitoring to detect new neurological deficits was 99.0% (102/103) for the entire cohort, and 100% (11/11) for those showing significant changes at the moment of positioning. Overall, only 1 patient, with non-significant SEP attenuation, experienced a new postoperative deficit, yielding a 0.97% rate of false negatives.Conclusion: Among patients undergoing surgery for cervical myelopathy, 10.7% showed alarming electrophysiological signal changes at the time of positioning. Objetivo: Evaluar la incidencia de alteraciones neurofisiológicas intraoperatorias graves en el momento del posicionamiento del paciente, y la efectividad de la recolocación del cuello para revertir dichos cambios en los pacientes que se intervienen de mielopatía cervical.Material y métodos: Se empleó una monitorización intraoperatoria multimodal (potenciales evocados sensoriales [PES], motores [PEM] y electromiografía) antes y después de colocar al paciente en posición, en una cohorte de 103 pacientes consecutivos operados de mielopatía cervical. Se consideraron cambios significativos (de alarma): una disminución >50% de la amplitud o un aumento >10% de la latencia de los PES, o la abolición o disminución >50-80% en amplitud de los PEM.Resultados: De los 103 pacientes (el 34,9% mujeres, mediana de edad: 54,5 años), a 88 se les realizó laminectomía (85,4%) y a 15 (14,6%) un abordaje anterior. En el momento del posicionamiento, ocurrieron alteraciones de señal en 44 pacientes (42,7%), aunque solo en 11 (10,7%) estas fueron significativas. La recolocación inmediata del cuello consiguió revertir la alteración de señal completa (n=6) o parcialmente (n=4), sin producirse déficits postoperatorios. El paciente en el cual la recolocación no consiguió restaurar los potenciales despertó con déficit neurológico añadido. La precisión (verdaderos positivos+verdaderos negativos) de la monitorización intraoperatoria para detectar déficits postoperatorios fue del 99% (102/103) para la cohorte completa y del 100% (11/11) para el subgrupo con alteraciones significativas. Globalmente, solo un paciente, que mostró cambios no significativos, despertó con nuevo déficit neurológico (0,97% de falsos negativos).Conclusión: El 10,7% de los pacientes intervenidos de mielopatía cervical mostraron cambios neurofisiológicos de alarma en el momento del posicionamiento quirúrgico. La inmediata recolocación del cuello revirtió dichos cambios (completa o parcialmente)(AU)

No significant differences in terms of complications or radiological variables were observed between the 2 techniques.

Resumen La fibrosis epidural es un hallazgo clínico y radiológico el cual se encuentra relacionado directamente en su génesis con los procedimientos quirúrgicos realizados en la columna vertebral. Esta patología es la causa más frecuente del síndrome de cirugía fallida de columna, manifestada clínicamente como un dolor equiparable en severidad al presentado previo a la intervención quirúrgica realizada. La fibrosis epidural es parte del proceso de cicatrización usual de los tejidos circundantes de la columna vertebral, en otras palabras, es esperable que esta suceda, sin embargo, solo una mínima cantidad de las personas intervenidas desarrollan clínicamente la sintomatología. La importancia del análisis de esta patología radica en que estas valoraciones son ingresadas al Departamento de Medicina Legal por asunto de reagravación de alguna secuela ocasionada por algún evento traumático. Es imprescindible conocer a profundidad los diferentes aspectos etiológicos, fisiopatológicos, clínicos, radiológicos y terapéuticos que involucran esta patología vertebral, así como, el respectivo análisis médico legal en este tipo de casos, tomando en consideración todos los elementos de juicio que permitan relacionar este hallazgo con algún hecho denunciado. En el presente artículo se realizará un análisis médico legal de un caso de reagravación por fibrosis epidural valorado en el Departamento de Medicina Legal de Costa Rica, donde el peritado interpone una denuncia contra el ente asegurador al ser rechazada la posibilidad de reapertura del riesgo laboral presentado.


Abstract The epidural fibrosis is a clinical and radiological finding that is related with the surgical procedures performed on the spine. This pathology is the most frequent cause of failed back syndrome or post laminectomy syndrome, that is clinically manifested by an acute pain comparable in severity with the surgical intervention. Epidural fibrosis is part of the usual healing process of the surrounding tissues of the spine, in other words, it's to be expected as a result of this procedures, nevertheless, only a small number of people who have been intervened, actually develop the symptoms clinically. The importance of this pathology lies in the fact that these assessments are attended in the Legal Medicine Department due to the aggravation issues of any sequel caused by a traumatic event. It is essential to know such things as the etiology, pathophysiology, clinical manifestations, radiological findings, and the therapeutic aspects that involve this vertebral disease, as well as, the respective forensic analysis, in order to use all the elements of judgment that allows to relate what is documented with the denounced findings.

Objetivo La laminoplastia «open door» es una técnica ampliamente utilizada para el tratamiento de la mielopatía cervical multinivel. A pesar de presentar resultados funcionales y radiológicos satisfactorios a largo plazo, la parálisis de C5 postoperatoria continúa siendo una complicación severa e invalidante con una incidencia variable en la literatura. El objetivo del presente trabajo es describir e ilustrar la técnica quirúrgica paso a paso con el agregado de la foraminotomía unilateral C4-5, y evaluar los resultados obtenidos hasta el momento, haciendo especial énfasis en la parálisis de C5. Material y métodos Estudio retrospectivo de 20 pacientes intervenidos por mielopatía cervical mediante la técnica de laminoplastia cervical «extendida» con foraminotomía unilateral, para la cual se detallan los pasos. Resultados Entre enero de 2013 y abril de 2019 se trataron 20 pacientes con «laminoplastia cervical extendida». Un solo paciente agregó déficit de C5 postoperatorio (5%). El porcentaje de recuperación del Japanese Orthopaedic Association score (JOA modificado) postoperatorio fue del 54,5%, siendo similar a lo observado en otras series. Conclusión Se desarrolló e ilustró la técnica de laminoplastia cervical «extendida» con foraminotomía unilateral de C4-5 para la prevención de la parálisis de C5. Se analizaron los resultados, y se obtuvo una incidencia de parálisis de C5 coincidente con el menor porcentaje reportado en la literatura. Sería de utilidad un estudio prospectivo y aleatorizado para valorar el rol de la foraminotomía preventiva C4-5 unilateral (AU)

Material and methods Retrospective study of 20 patients operated on for cervical myelopathy using the “extended” laminoplasty technique, which is described step by step. Results Between January 2013 and April 2019, 20 patients were operated on using the extended laminoplasty technique. Only one patient (5%) presented postoperative C5 palsy. The postoperative recovery rate of the modified JOA (Japanese Orthopaedic Association) score was 54.5%, similar to that observed in other series. Conclusion The extended cervical laminoplasty technique with unilateral C4-5 foraminotomy was developed and demonstrated for the prevention of C5 palsy. The results were analysed and an incidence of C5 palsy coinciding with the lowest percentage reported in the literature was obtained. A prospective randomised study would be useful to assess the role of preventive unilateral C4-5 foraminotomy (AU)

Despite the satisfactory functional and radiological results of this technique, postoperative C5 palsy is still a severe and disabling complication with a variable incidence in the literature. The objective of this article is to describe and demonstrate the surgical technique step by step with the addition of unilateral C4-5 foraminotomy and to evaluate the results obtained to date, with special emphasis on C5 palsy. MATERIAL AND METHODS: Retrospective study of 20 patients operated on for cervical myelopathy using the "extended" laminoplasty technique, which is described step by step. RESULTS: Between January 2013 and April 2019, 20 patients were operated on using the extended laminoplasty technique. Only one patient (5%) presented postoperative C5 palsy. The postoperative recovery rate of the modified JOA (Japanese Orthopaedic Association) score was 54.5%, similar to that observed in other series. CONCLUSION: The extended cervical laminoplasty technique with unilateral C4-5 foraminotomy was developed and demonstrated for the prevention of C5 palsy. The results were analysed and an incidence of C5 palsy coinciding with the lowest percentage reported in the literature was obtained. A prospective randomised study would be useful to assess the role of preventive unilateral C4-5 foraminotomy.

OBJECTIVE: To evaluate the incidence of significant intraoperative electrophysiological signal changes during surgical positioning, and to assess the effectiveness of head and neck repositioning on the restoration of signals, among patients undergoing surgery for cervical myelopathy. MATERIAL AND METHODS: We used multimodal intraoperative monitoring (somatosensory [SEP] and motor evoked potentials [MEP] and spontaneous electromyography) before and after patients' positioning in a consecutive cohort of 103 patients operated for symptomatic cervical myelopathy. Significant changes were defined as>50% attenuation in amplitude or>10% increase in latency of SEP, or abolishment or 50-80% attenuation of MEP. RESULTS: Out of 103 patients (34.9% female, median age 54.5 years) 88 underwent laminectomy (85.4%) and 15 (14.6%) anterior approach. At the time of positioning, signal alterations occurred in 44 patients (42.7%), yet only 11 patients (10.7%) showed alarming changes. Immediate neck repositioning of these resulted in complete (n=6) or partial (n=4) restoration of potentials, yielding no postoperative deficits. The patient in which signals could not be restored after repositioning resulted in added postoperative deficit. The accuracy (true positives plus true negatives) of monitoring to detect new neurological deficits was 99.0% (102/103) for the entire cohort, and 100% (11/11) for those showing significant changes at the moment of positioning. Overall, only 1 patient, with non-significant SEP attenuation, experienced a new postoperative deficit, yielding a 0.97% rate of false negatives. CONCLUSION: Among patients undergoing surgery for cervical myelopathy, 10.7% showed alarming electrophysiological signal changes at the time of positioning. Immediate repositioning of the neck resulted in near always restoration of potentials and avoidance of added neurological damage. Complete or partial restoration of potentials after repositioning yielded no postoperative deficits.

Anterior sacral meningocele is a protrusion of the dural sac through a bone defect in the sacrum anterior wall. It can occur in isolation or be associated with other malformations, such as the Currarino syndrome, which is part of the classic triad together with hemisacrum and anorectal malformation, although it has great variability. This is the case of an adult woman with chronic constipation, recurrent urinary infection and marked abdominal distension, diagnosed with giant anterior sacral meningocele and incomplete Currarino syndrome. The patient underwent a sacral laminectomy and thecal sac ligation at the level of the bone defect. This article describes this rare type of occult spinal dysraphism as well as the surgical technique used.

OBJECTIVE: The open-door laminoplasty technique is widely used in the treatment of multilevel cervical myelopathy. Despite the satisfactory functional and radiological results of this technique, postoperative C5 palsy is still a severe and disabling complication with a variable incidence in the literature. The objective of this article is to describe and demonstrate the surgical technique step by step with the addition of unilateral C4-5 foraminotomy and to evaluate the results obtained to date, with special emphasis on C5 palsy. ABSTRACT Objective: To compare the postoperative outcomes of transforaminal intersomatic lumbar arthrodesis with structured iliac bone autograft and PEEK device. Methods: The total of 93 medical records of patients undergoing transforaminal intersomatic fusion between January 2012 and July 2017 with at least 1 year of postoperative follow-up, with complete medical record, containing clinical file and radiological exams, were reviewed. Results: From the medical records evaluated, 48 patients underwent the procedure with structured iliac autograft (group 1) and 45 with PEEK device (group 2). There was an improvement in functional capacity in both groups (p < 0.001), however there was no difference when comparing them (p = 0.591). Conclusion: The postoperative clinical and radiological results of lumbar arthrodesis with TLIF technique, using a structured iliac bone autograft compared to a PEEK device, were similar. RESUMO Objetivo: Comparar os desfechos pós-operatórios da artrodese lombar intersomática transforaminal com autoenxerto ósseo ilíaco estruturado e dispositivo em poliéter-éter-cetonico-polímero (PEEK). Métodos: Foram revistos 93 prontuários de pacientes submetidos à fusão intersomática transforaminal entre janeiro de 2012 e julho de 2017 com pelo menos um ano de acompanhamento pós-operatório, com prontuário completo, contendo ficha clínica e exames radiológicos. Resultados: Dos prontuários avaliados, 48 pacientes foram submetidos ao procedimento com autoenxerto ilíaco estruturado (Grupo 1) e 45 com dispositivo em PEEK (Grupo 2). Houve melhora da capacidade funcional em ambos os grupos (p < 0,001), no entanto não houve diferença ao compará-los (p = 0,591). Conclusão: Os resultados clínicos e radiológicos pós-operatórios da artrodese lombar na técnica Fusão Intersomática Lombar Transforaminal (TLIF) utilizando autoenxerto ósseo ilíaco estruturado, em comparação com o uso de dispositivo em PEEK, foram semelhantes. RESUMEN Introducción: Los tumores de columna vertebral son neoplasias poco comunes que pueden ser de origen primario o secundario. Las de origen primario son las que se presentan con menor recurrencia clínica, y se caracterizan generalmente por manifestaciones de dolor a nivel local y en menor proporción, por afectación neurológica. Objetivo: Describir el proceso de diagnóstico de un paciente con tumor de columna lumbar, el tratamiento recibido y su evolución. Caso clínico: Paciente de 77 años de edad con cuadro clínico consistente en dolor lumbar, paresias y parestesias. Se realizaron ayudas diagnósticas como radiografía y tomografía, en las que se evidenció una extensa lesión tumoral de la columna lumbar, valorada por cirugía de columna. Se realizó laminectomía descompresiva a nivel de L4 bilateral, artrodesis de T12 a L5 y se tomó biopsia abierta del tumor de la columna. Después de la cirugía la paciente presentó mejoría de la sintomatología. El informe anatomopatológico del tumor evidenció un tumor maligno indiferenciado. Conclusiones: El diagnóstico de tumor en columna lumbar se basa en evidencia clínica, imágenes diagnósticas, biopsia e inmunohistoquímica; una vez realizado el diagnóstico, el tratamiento debe incluir una combinación de quimioterapia, radioterapia y cirugía. La tomografía axial computarizada se utiliza como ayuda quirúrgica. Durante el procedimiento quirúrgico es pertinente realizar la resección de la lesión y obtener una confirmación histológica para definir el tratamiento antineoplásico y la malignidad de dicho tumor. El tratamiento oportuno reduce el riesgo de daño neurológico y mantiene la estabilidad mecánica espinal(AU)


ABSTRACT Introduction: Spinal tumors are rare neoplasms that can be of primary or secondary origin. Those of primary origin are those that present with less clinical recurrence, and are generally characterized by manifestations of local pain and to a lesser extent, by neurological involvement. Objective: To describe the diagnostic process of a patient with lumbar spine tumor, the treatment received and the evolution. Clinical report: We report a case of a 77-year-old patient, with a clinical situation of low back pain, paresis and paresthesias. Diagnostic aids such as radiography and tomography were performed, which showed an extensive tumor lesion of the lumbar spine, it was assessed for spinal surgery. Decompressive laminectomy was performed at the bilateral L4 level, arthrodesis from T12 to L5, and an open biopsy of the spinal tumor was taken. The patient had improvement in symptoms after surgery. The pathological report of the tumor showed an undifferentiated malignant tumor. Conclusions: The diagnosis of lumbar spine tumor is based on clinical evidence, diagnostic images, biopsy and immunohistochemistry. Once the diagnosis is made, treatment should include a combination of chemotherapy, radiation therapy, and surgery. Computerized axial tomography is used as a surgical aid. During the surgical procedure, it is pertinent to resect the lesion and to obtain histological confirmation to define the antineoplastic treatment and the malignancy of said tumor. Prompt treatment reduces the risk of neurological damage and maintains spinal mechanical stability.

La estenorraquis lumbar es la disminución del volumen del canal raquídeo en la columna. Es la causa más frecuente de dolor lumbociático y lumbocrural en población mayor de 60 años. Existen múltiples causas. La más común es la degenerativa que consiste en una disminución del canal secundario a varios factores como protrusión del disco intervertebral, hipertrofia y abombamiento del ligamento amarillo, engrosamiento de la capsula articular y por osteofitos. Los síntomas principales son el dolor lumbar irradiado a extremidades inferiores tipo lumbociática o lumbocruralgia y la claudicación neural intermitente. Generalmente no hay déficit motor ni sensitivo. La resonancia magnética es el estudio de elección para el diagnóstico, pero debe complementarse con radiografías y con frecuencia con tac para una mejor valoración de la patología. El tratamiento inicial siempre es conservador, que incluye uso de antiinflamatorios, analgésicos, relajantes musculares, kinesioterapia e infiltraciones de columna (epidurales o radiculares). La mayoría de los pacientes responden satisfactoriamente a este manejo. El tratamiento quirúrgico está indicado a los pacientes que presentan un dolor intenso, que afecta su calidad de vida y que no mejora con el tratamiento conservador. Consiste en la descompresión quirúrgica de las raíces lumbares a través de laminectomía. En ocasiones se recomienda complementar la descompresión con artrodesis específicamente cuando la estenorraquis se asocia a otras condiciones como espondilolistesis degenerativa, escoliosis, desbalance sagital o coronal, inestabilidad segmentaria y en enfermedad del segmento adyacente.


Spinal lumbar stenosis is a narrowing of the spinal canal in the lumbar spine. Is the most frequent cause of sciatic pain or cruralgia in the elderly population over 60 years old. Exist many etiologies. The most common is degenerative caused by many factors including bulging disc, hypertrophy of flavum ligament as well as facet capsule thickening and by osteophytes formation. The main symptoms of spinal stenosis are radicular pain and neurogenic claudication. Lumbar flexion usually improves the symptom and deambulation often worsens the pain. Most of the time neurologic examination is normal. The gold standard study is mri but is necessary to complement with x rays and ct to obtain a better evaluation. Initial treatment is always conservative and consists in nsaid's, muscle relaxants, physical therapy and steroid spinal injections. Most of the patients respond well to this treatment. Surgical treatment is indicated when the patient has a severe pain, quality of life is miserable and conservative treatment fails. Decompression is the gold standard surgical treatment. The addition of an arthrodesis or spinal fusion is recommended in degenerative spondylolisthesis, scoliosis, sagital or coronal imbalance, deformity correction, recurrent spinal stenosis and in cases of adjacent level disease problem.

ABSTRACT Objective To identify the incidence and possible risk factors associated with rod breakage in patients who underwent vertebral column resection (VCR) or pedicle subtraction osteotomy (PSO) to treat complex deformities of the spine. Methods Retrospective analysis of a series of 32 patients operated from 2014 to 2018 in a single center. The patients were analyzed for demographic (sex, age), biometric (BMI), radiographical (pre- and postoperative angular variations), and surgical (arthrodesed and osteotomized levels) characteristics. Descriptive analyses were performed for the numerical variables (mean, standard deviation, maximum, median, and minimum) and frequency analysis was performed for the categorical variables. Logistic regression analysis was performed for the dependent variable "rod breakage", using a stepwise technique to select the variables for the best model, assuming statistical significance of 0.05. Results Of the 32 patients selected, rod breakage occurred in 34.4%. Mean age was 36.6 years (± 19.8), ranging from 10 to 74 years, and the mean BMI was 25.1 (±6.0). Most patients were subjected to VCR (75.0%), were males (56.2%) and did not smoke (90.6%). Logistic regression analysis showed that "arthrodesed levels" were was positively associated with rod breakage (OR 1.72; CI95%: 1.13-3.10; p<0.05). The other factors were not associated with breakage. Conclusion Rod breakage is a frequent complication after three-column osteotomy, especially in long constructions. RESUMO Objetivos Identificar a incidência e possíveis fatores de risco relacionados à quebra de hastes em pacientes submetidos à ressecção colunar vertebral (RCV) ou osteotomia de subtração pedicular (OSP) para o tratamento cirúrgico de doenças complexas da coluna vertebral. Métodos Análise retrospectiva de uma série de 32 pacientes operados de 2014 a 2018, em um único centro. Os pacientes foram avaliados quanto às características demográficas (sexo, idade), biométricas (IMC), radiológicas (variação angular antes e após a correção) e cirúrgicas (níveis artrodesados, níveis osteotomizados). As análises descritivas foram realizadas para as variáveis numéricas (média, desvio-padrão, máximo, mediana e mínimo) e, para as variáveis categóricas foi realizada a análise da frequência. Realizou-se análise de regressão logística para a variável dependente "quebra", utilizando a técnica stepwise para seleção das variáveis que compõem o melhor modelo, considerando o nível de significância de 0,05. Resultados Foram selecionados 32 pacientes; a proporção de quebra foi de 34,4%. Observou-se que a média de idade foi de 36,6 anos (± 19,8 anos), distribuindo-se entre 10 e 74 anos, e o IMC médio foi 25,1 (± 6,0). Verificou-se que a maior parte dos indivíduos foi submetida a técnica RCV (75,0%), era do sexo masculino (56,2%) e não fumava (90,6%). Na análise de regressão logística, "níveis artrodesados" associou-se positivamente à quebra (OR 1,72; IC95%; 1,13-3,10; p < 0,05). Os outros fatores não se associaram à quebra. Conclusão A quebra de hastes é uma complicação frequente das osteotomias das três colunas, principalmente, em construções longas. Nível de evidência III; Estudo Retrospectivo.

Métodos Análisis retrospectivo de una serie de 32 pacientes operados desde 2014 hasta 2018, en un único centro. Los pacientes fueron evaluados cuanto a las características demográficas (sexo, edad), biométricas (IMC), radiológicas (variación angular antes y después de la corrección) y quirúrgicas (niveles artrodesados, niveles osteotomizados). Los análisis descriptivos fueron realizados para las variables numéricas (promedio, desviación estándar, máximo, mediana y mínimo) y, para las variables categóricas fue realizado el análisis de frecuencia. Se realizó análisis de regresión logística para la variable dependiente «rotura¼, utilizando la técnica stepwise para selección de las variables que componen el mejor modelo, considerando el nivel de significancia de 0.05. Resultados Fueron seleccionados 32 pacientes; la proporción de rotura fue de 34,4%. Se observó que el promedio de edad fue de 36,6 años (± 19,8 años), distribuyéndose entre 10 a 74 años, y el IMC promedio fue de 25,1 (± 6,0). Se verificó que la mayor parte de los individuos fue sometida a la técnica RCV (75,0%), era del sexo masculino (56,2%) y no fumaba (90,6%). En el análisis de regresión logística, los "niveles artrodesados" se asociaron positivamente a la rotura (OR 1,72; IC 95%; 1,13-3,10; p <0,05). Los otros factores no se asociaron a la rotura. ABSTRACT Objective: Epidural fibrosis is one of the main reasons for requiring repeated surgical intervention. Our objective was to compare the effect of Platelet Rich Plasma (PRP) on the development of epidural fibrosis with collagen dural matrix and free autogenous fat graft. Methods: Male rats were separated into 3 groups. Laminectomy was implemented on the rats and epidural fat pad was placed in the first group (n = 7); equal size of collagen dural matrix was applied in the second group (n = 7); a single dose of PRP was applied in the third group (n = 7). Results: Epidural fibrosis was more common in the group that collagen dural matrix was applied when compared the ones that PRP was applied. PRP group presented better values in preventing epidural fibrosis when compared to the fat pad group, however this difference was not statistically significant. Conclusion: PRP is a material that can be easily obtained from the very blood of patients and at an extremely low cost; the main clinical relevance of our study is that the PRP might be an efficient material for better clinical results after laminectomy surgery due to its tissue healing and epidural fibroris preventing potentials. RESUMO Objetivo: A fibrose epidural é uma das principais razões que motiva intervenções cirúrgicas repetidas. O objetivo deste estudo foi comparar o efeito do plasma rico em plaquetas (PRP) no desenvolvimento de fibrose epidural com matriz de colágeno e enxerto de gordura autógena. Métodos: Ratos machos foram separados em 3 grupos. A laminectomia foi aplicada nos ratos e gordura epidural foi colocada no primeiro grupo (n = 7); matriz de colágeno de tamanho igual foi aplicada no segundo grupo (n = 7); uma dose única de PRP foi aplicada no terceiro grupo (n = 7). Resultados: A fibrose epidural foi mais comum no grupo em que a matriz de colágeno foi aplicada, quando comparada aos animais do grupo PRP. O grupo PRP apresentou os melhores valores na prevenção da fibrose epidural quando comparado ao grupo enxerto de gordura, porém a diferença não foi estatisticamente significante. Conclusão: PRP é um material de fácil obtenção do sangue dos pacientes e a baixo custo; a principal relevância clínica de nosso estudo é que o PRP pode ser um material eficiente para obter melhores resultados clínicos após a laminectomia devido à sua cicatrização tecidual e potencial de prevenção de fibrose epidural. OBJECTIVE: To assess the effectiveness and safety of vancomycin powder as surgical site infection (SSI) prophylaxis in posterior bilateral elective spinal surgery. MATERIALS AND METHODS: Single-center quasi-experimental pre and postintervention comparative cohort study. The post-intervention group received standard intravenous antibiotic prophylaxis plus 1g of vancomycin powder into the surgical field before wound closure, and the pre-intervention group only the intravenous prophylaxis. RESULTS: 150 patients were included in each group. Twelve SSI (7 superficial and 5 deep) occurred in the post-intervention group and 16 SSI (7 superficial and 9 deep) in the pre-intervention group. The risk of deep SSI decreased from 6.0% to 3.3% (OR 0,54, 95%CI 0.17-1.65, p=0.411) with vancomycin powder. The percentage of deep SSI due to gram negative-positive germs were 80%-20% and 33%-67% for the post- and pre-intervention groups, respectively (p=0.265). No local or systemic adverse effects occurred attributable to vancomycin powder. CONCLUSION: In posterior elective spinal surgery, prophylaxis with vancomycin powder did not result in a significantly reduced incidence of superficial and deep SSI. There was a trend towards a higher incidence of deep SSI caused by gram negative microorganisms among those treated with vancomycin.

BACKGROUND: Streptococcus agalactiae is an uncommon microorganism that causes spinal epidural abscess (SEA) and usually affects individuals with a predisposing condition or potential source of infection. CASE DESCRIPTION: We present the case of an immunocompetent 53-year-old patient with an unremarkable past medical history who developed progressive low extremity weakness, bowel and bladder dysfunction and genital sensory impairment. A neurological exam on admission revealed flaccid proximal paraparesis, T10 sensory level, atonic anal sphincter and normal myotatic reflexes. Urgent neuroimaging showed a large thoracic epidural spinal abscess. Laminectomy and abscess drainage were immediately performed and systemic antibiotic treatment was initiated. Abscess cultures revealed Streptococcus agalactiae. After an exhaustive workup no predisposing factors or local or systemic source for the infection were found. CONCLUSIONS: We report a singular case of spinal epidural abscess caused by Streptococcus agalactiae in a healthy patient with no predisposing factors. This case also highlights the importance of an early diagnosis and treatment to obtain a better neurological outcome.
